Job Description

The Peaks Care Center is seeking a Care Navigator to join our team!

The Care Navigator is a vital part of our community. You will be managing a diverse caseload of skilled nursing residents. Your role focuses on providing comprehensive medically related social services, ensuring residents attain or maintain their highest practicable physical, mental, and psychosocial well-being. You will also be responsible for navigating residents to the appropriate services, acting as a connector to ensure their needs are met through various community resources and support systems and assisting residents with applying for Medicaid as well as managing the facilities Medicaid pending process. This position will support both the Social Services Director and the Business Office Manager.

Responsibilities:

  • Manage the Medicaid application process.
  • Ensure timely follow up of Medicaid Pending application, documents, status, etc. with appropriate state agencies.
  • Support the rehab unit and assist with admissions and discharges.
  • Coordinate delivery of services to managed care and Medicare residents in collaboration with the facility’s team members.
  • Complete timely and thorough communication with managed care/insurance case managers to coordinate certification and stay programs.
  • Participate in the orientation of new residents/family members to the facility.
  • Responsible for scheduling care conferences with residents, family members, and staff.
  • Participate in discharge planning, development and implementation of social care plans and resident assessments.
  • Facilitate resident discharges including arranging post discharge services.
  • Assists residents and their families with resources pertaining to discharge planning and post discharge planning.
  • Participate in developing, implementing, and periodically updating the resident’s written discharge plan.
  • Ensure that social services components of the MDS are completed and signed on a timely basis.

Collaborates with Social Worker, Unit Coordinator, Business Office Manager, and other facility departments.

Qualifications:

  • English literate and possess strong written and verbal communication skills.
  • Sound judgment and problem-solving skills.
  • Is accurate, dependable, neat and thorough in assigned tasks.
  • Ability to develop and maintain good working relationships with co-workers and other departments.
  • Ability to work under minimal supervision and make decisions independently.
  • Strong ability to multitask and strong attention to detail.

Education Requirements:

  • High School Diploma or GED required.
  • Bachelor's degree from four-year college or university preferred.
  • Minimum of 2 years related experience required.
  • Experience in Skilled Nursing, Assisted Living and/or Multi level care facilities preferred.
