Virtual Care Coordinator
Location: New York, NY 10017
Duration: 13 Weeks
Shift: 3:00 PM – 11:00 PM
Pay Rate: $25 – $27/hour
Summary:
"Navitas Healthcare, LLC" is seeking Virtual Care Coordinator for an exciting job in New York, NY.
Key Duties:
Monitor patients remotely and escalate clinical concerns to nursing staff.
Support telehealth sessions, troubleshoot technology, and ensure workflow efficiency.
Maintain and analyze program data, quality metrics, and utilization reports.
Train staff on telehealth platforms and documentation standards.
Participate in quality reviews, policy development, and workflow optimization.
Requirements:
Minimum 3 years of direct patient care experience .
Hands-on experience with telehealth/remote monitoring and EMR systems (Epic preferred) .
Strong communication, technical troubleshooting, and analytical skills.
Associate or Bachelor’s degree in a health-related field preferred.
